Tip It Forward Clinic Policies and procedures


INTAKE PROTOCOL

Client Information, Informed Consent, and Waiver forms must be completed prior to or at the first visit and will be provided upon scheduling.

Payment

Payment is due on the day of the session unless otherwise arranged. We accept Visa, American Express, MasterCard, Discover, cash and checks. A $35 fee will be assessed for each returned check. Payments for all services are non-refundable.

Sessions

If you show up late to your appointment, you will be charged for the amount of time you scheduled. For consistency and out of respect for everyone's schedule, sessions start on time and end on time. Please make an effort in being prompt to the best of your abilities.
Appointments generally book about two or three weeks in advance. Please keep this in mind if you would like to maintain a particular schedule of appointments.

Last Minute Appointment Cancellations and No-Show Policy

Please notify us over 24 hours in advance to cancel or reschedule. Because your appointment is set specifically for you and booked well in advance, a cancellation results in time that your therapist could have spent healing others. If less than 24 hours notice is provided to cancel your appointment or you do not show up, you will be charged 50% of the fee of the booked appointment. If you arrive late to your appointment, we will try to accommodate you, but your treatment may be shortened to maintain the schedule in consideration of other booked appointments. 

CONFIDENTIALITY

The privacy of your personal information is important to our clinic. We are committed to collecting, using and disclosing personal information responsibly and only to the extent necessary for the goods and services we provide.

I understand that Tip It Forward will collect personal information about me in an effort to ensure that my care is appropriate. This information is stored in a secure database and will not be shared with anyone outside of Tip It Forward, unless legally required to do so. 

PACKAGES & DISCOUNTS

Multi-Session Discounts

If you would like to purchase 3 or more sessions at a time, a 10% discount will be applied to your session fees when payment is provided at the time of booking and scheduling. Please contact us directly if you are interested in booking more than six sessions for other potential discounts as we often have seasonal promotions. Gift Certificates

Gift Certificates expire one year from the date of purchase. Gift Certificates must be present at the time of your appointment. We do not honor lost or stolen Gift Certificates. Gift certificates are nonrefundable, and cannot be redeemed for cash. A 24-hour notice is required to cancel your appointment. Last minute cancellations and no shows forfeit the certificate. No compensation will be given. If you call to schedule your appointment before the expiration date, but no slot is available before the card expires, you will be able to schedule a session within 3 weeks from the expiration date. If your paper gift certificate has officially expired,, we will offer you 50% off the service listed on the gift certificate.  

Delayed Onset Muscle Soreness (DOMS)

I understand that I may experience delayed onset muscle soreness from treatment, and that I may experience temporary discomfort as a result. I understand that the soreness I may experience is also affected by the level of hydration, mineral balance, movement, and underlying medical conditions. DOMS makes your muscles feel sickly, sore, and weak. It is that distinctive muscle pain that nearly everyone experiences after intense or unfamiliar exercise, often peaking as long as a day or two later. The symptoms often begin after sleeping, and then continue for 24 to 72 hours. If your soreness continues after 72 hours, please let your practitioner know.

I understand that Tip It Forward practitioners do not prescribe medical treatment or pharmaceuticals, and therefore is not a substitute for medical treatment.

Some of the services that I will receive will include physical touch. All touch is strictly professional and conducted by professional licensed therapists and certified movement educators.

You may feel emotional during a session.  If you feel the need to talk or end the session, you may do so safely and comfortably. However your therapist is not a counselor, and will not provide professional counsel. 

Both the therapist and I have the right, at any time, to terminate the session. I have the right to request that any procedure be changed, stopped, or simply not performed. 

I understand if I experience any unusual discomfort during my sessions it is my responsibility to inform the therapist so that they can adjust the pressure or technique being used.  I agree to update the therapist on changes in my health and understand that there will be no liability on the therapists' part should I forget to do so. I have provided my therapist with accurate information about my medical conditions and history and hereby agree and consent to the recommended therapy. 

With all this in mind, I agree to have treatment and hold the practitioner and Tip It Forward harmless for any problem that might seem to arise as a result of a therapy session.

I have discussed the risks, benefits, and alternatives to the suggested treatment and understand that there are no guarantees of success. I have had the opportunity to ask questions and they have been answered to my satisfaction, such that I have adequate and informed understanding of the services performed by Tip It Forward practitioners.
